Dear Sir,

I desire to be enrolled as a Member of your Association and remit herewith a sum of Rs 10,500/- (Rupees Ten Thousand Five Hundred only), which is Admission Fee Rs. 500/-and Rs 10,000 (Rupees Ten Thousand only) as ONE TIME SUBSCRIPTION in lieu of the annual subscription.

I hereby declare that I subscribe to the aims and objects of the Association and agree to abide by its Rules and Regulations in force from time to time. My name, address and other particulars are given below to enable you to consider my application.

Application Shall be Scrutinised for information provided & membership shall be approved by the president, KPA.
If for any reason membership does’nt get approved membership fee submitted shall be refunded.
Application fee is non refundable.
